Worker privacy notice and consent form
The worker privacy notice and consent form explains how the Department of Employment and Workplace Relations, and the people and departments it works with, collect and use your personal information. They follow the rules in the Privacy Act 1988 to keep your information safe.
An information sheet and a set of frequently asked questions (FAQs) have been developed to help workers understand the importance of re-signing the updated privacy notice and consent form.
In addition, a separate set of FAQs has been prepared to assist employers. A confirmation form is also available on this page, which employers are requested to complete.
